Captioning Request Form

Disability Services for Students is committed to providing captions to students with disabilities and in keeping compliant with the CSU Accessible Technology Initiative and federal law (Section 508).

This form is for instructors and staff to request media captioning. Captioning requests should be made three-four weeks in advance.  If your media is in a foreign language, the captioning process will take an additional week to process your request.
